Comparative study of operative methods for peptic ulcer

Abstract
Various controversies attend the treatment of gastric and duodenal ulcer; and it will not be
wise to formulate best operative procedure for these lesions because the procedures may depend
responsible visual status of the lesion and the patient¢¥s condition.
This article is a summary of the result of surgical. treatment of gastric and duodenal ulcer
followed up for two to fourteen years 120 out of 1000 patients who had surgery of four different
methods applied according to the individual patient¢¥s condition are followed z up.
(Subtotal gastrectomy with Billroth I; Billroth .II; transabdomir-al trunkal - vagotomy - with
drainage; hemigastrectomy with selective vagotomy and jejunal transposition (J- T).
1. Amount of post-operative weight loss was highest in the group of B II and, lowest in J-T. 2. Iron defficiency anemia was remarkably high in group of J-T and low in vagotomy and
drainage group. However, there is no significant difference between Billroth I. and IIgroup.
3. Total serum protein is most remarkably decreased in the group of vagotomy and drainage
group but not in J-T group.
4. Postop acid reduction rate of both basal and M.A.O was lowest in J-T and Billroth II .groups highest in the group of vagotomy and drainage group.
5. Diarrhea was observed approximately 42.9% in vagotomy and drainage group in mild degree and non of these patients complained any disturbance of daily life. As a summary it should be stated that the numbers of the followed up patients are so limited and period of follow up are not long enough that continuous further study is needed.
